Important Booking Information
Please enter full real names email and phone contact. Do not fill in any requested information using nicknames. Please enter correct address where services will begin to end, Airbnb Apt or Hotel Home.
It is customer responsible for pure recovery company parking at Airbnb or Hotel. All pulled tickets will not be paid by pure recovery and company.
Pure recovery doesn’t split service hours of booked services. All times and private service packages are set.
If any patient need to reschedule please contact our office to speak directly to a rep to ensure there’s available open slots to reschedule date and services.
Patients that has booked with pure recovery that have any questions we are available to answers any questions by phone 5 days a week M-F 8am to 5pm. In the case of any life threatening emergencies please contact 911 for assistance.

All pure recovery services requires a deposit and booking fee. Taxes will be included in services prices. All deposits goes towards paid services.
